Build the exact form your company needs
Forms are built with a fully custom, no-third-party-library drag-and-drop Form Builder — titles, fields (text, select/rating, textarea, checkboxes, radios), spacers, dividers, and page breaks, all configurable per Type + Year. Forms are saved once and reused across every employee in that campaign.
A review experience built around sequencing
- The form renders with all previously-saved answers already filled in and locked for fields outside the reviewer’s assigned step (visibly greyed out)
- Only the fields belonging to the reviewer’s own step are editable
- A “Show Empty Fields” toggle highlights anything not yet answered
- Auto-save runs 800ms after every change, with a “Last saved at [time]” indicator — nobody loses work to a closed tab
- Digital signatures (reviewer and employee) are captured and write-once — once signed, a signature can’t be silently overwritten
- Contextual help tooltips can be attached to any field or section title
Once submitted, a reviewer or admin can generate a branded PDF of the complete review — all sections, all reviewers’ answers, and both signatures — for permanent record-keeping or sharing outside the system.
Every reviewer sees a form that’s already pre-filled with what came before them, locked everywhere except their own section. When it’s done, one click produces a clean, signed, professional PDF.
